Nothing special on the eForm 1. You list "MFI" as "Manufacturer Code," to indicate "Manufactured on Form 1," and you can choose from a large list of already-submitted model names (some of them quite funny), or come up with your own.

The important/distinct things you provide for a Form 1 suppressor are Model, Caliber, and Serial Number. You assign all three yourself.
