List of Towns in MA for Degree of Difficulty with LTC

I remember seeing a few years ago a list of all towns in MA with a "degree of difficulty" score (red-orange-green) for obtaining an LTC. I can't seem to find it, and need info on Medford for family members. Could someone point me in the right direction?
