FA10 for an SBR

All-knowing NES:

What's the correct way to list the make and manufacturer of an SBR on an FA10? Should it be the make/manufacturer of the host or should it be the info from the Form 1 (if different)?

Whoever made the lower is still the manufacturer of the lower, you're just the maker of the SBR. I put the manufacturer/model from the original lower on the FA10 for mine and it hasn't been an issue. I'm sure if you put your name/trust name on the FA10 it would go through fine as well, they just care that you registered it.
