Kahr PM40 - Slide Locked Back

Back from the range this morning. I've got a PM40 that has been broken in (~350 rounds). Shooting a couple of mags of carry ammo. After 3 or 4 rounds, slide locked back with rounds still in the mag. Unable to get the slide forward no matter what I try.

Out of good ideas. Before I ship this back to Kahr, and advice from the NES universe ?

Are you able to remove the loaded mag? Might be slide stop spring or a broken mag follower.

From the Kahr website:

Q. My slide locks back after each round fired. What is wrong?

A. Either the slide stop spring is damaged or it is a broken follower. Please call 508-795-3919 and ask for replacements. Instructions for replacing the followers are in your Owner's Manual. The slide stop spring is held with a T6 torque screw.

From a gun test site:

I had the PM40 with stainless slide and night sights for about 10 months. Slide would lock back in the middle of a magazine (every 10 rounds on average), slide wouldn't lock back at the end of a magazine (occasionally), the rearward motion of the barrel would cause the feed ramp to contact the follower, so the followers would break at between 50-100 rounds, at one point the magazine itself came apart at the seam from being hammered by the feed ramp.

Another possibility is not reassembling the slide stop correctly and bending the spring after cleaning the gun:

I have a Karh compact 3.5" barrel pistol, .40. No problems whatever in well over 800 rounds. Disassembly is easy but reassembly requires careful attention to re-insertion of the slide stop so that the correct part of it goes UNDER the small spring on the slide. I apparently did not do that right during one of my cleanings and bent the spring which cause to slide to stay open after one shot. I called Kahr and they are sending me a new spring. Not a really big deal, easy to replace (I hope), but wondering if anyone else has had similar problems and suggestions as to how to avoid them.​

Not the mag, good there. Gun is empty/safe, mag drops without issue.

The slide stop issue has me thinking...I've followed this re-assembly procedure a few times now so I should know better...but I *did* just have this thing apart for a cleaning...hmmm.
