I am about to send in a Form 1 for an SBR that will be registered to my NFA trust that I created but I have some questions on the assignments page. Here do I only need to list out the lower that will be the host for the SBR or should I also list the upper that I plan on buying? It also has lines for 2 witnesses. Can these be family members or should they be independents? Do I also need the assignments page notarized? Thanks in advance for any advice.
