There are no restrictions listed there. Likely you used the wrong model number on page 1 and instead of rejecting your ass they noted it that way

I have had them bounced before for dumb stuff like that.
Its the main reason I i got ETF prints for the responsible parties on my trust.
Redoing the prints and mailing them in was a pain in the ass.

There is an area on the form to submit pictures of the weapon. Not sure exactly what to send them. What did you all submit?
I thought the engraving had to be done after the tax stamp was approved?Photo of the make and serial number plus photo of the engraving of your maker information (name or trust name and town).

So a picture of the serial number on the upper would suffice?You can engrave it before or after, though it needs to be engraved before you actually put it in SBR (or whatever the NFA item is going to be) configuration.
