When you file that form, is it only for the state you designate?
The form is filed for a date range and they ask for specific locations within the state but it is generally good for travel to that state.
Yeah they want what yer moving, the destination address / details, and if it's coming home to the original location. The form is here if you want to take a look:

5320.20 forms are pretty quick to come back - my most recent one came back just a couple of weeks after I sent it in, so if that is still the case you have plenty of time to get yours in before the pumpkin blast.
Lead time is somewhere in the 2-8 week timeframe from what they told me when I called this summer.

I was told a six week wait on the form 20's. One came back in two weeks, the other in eight. The eight was supposed to be a rush so perhaps that was the problem.

Oh no kidding, that's handy I guess. Is the Fax# commonly available? I didn't see it on the form.
Congrats on the M11/9 by the way .... I've still got my eyes out for a MG in 2016. Been busy with HK and AK SBR's.

It shoulders nice and gives a good enough cheek weld too. I am a fan, way better then that Troy H&K wannabe stock.
