And and all of these. A friend took a job in the Dallas area when his company relocated. His wife and daughter refused to move with him and for almost a year he'd come back up here a couple of times a month to visit. He quit that job and moved back after that, but would love to move back down here if he could.
In our case it as a combination of family, friends, and to a lesser extent proximity to doctors. It was almost ten years from when we started planning (or at least I did) until we were able to move.
I'd say that people who can quit jobs, leave loved ones behind, and have nothing that obliges them to stay in one place are "lucky," but they probably have difficult lives for other reasons.

Some stay because of work, family or proximity to doctors/hospitals. I would have left years ago, but the wife won't go.
