THE CHAMBER IS NOT EMPTY UNLESS YOU CAN SEE IT'S EMPTY.

Any fool that relies on racking a slide or bolt to verify an empty chamber is just begging to shoot themselves or someone else.

This is a bit different:


Shortly after 6 p.m., a cannon onboard Old Ironsides misfired and seemed to have lobbed a projectile at a building on the grounds of the Charlestown Navy Yard.

Boston Police were requested to help find the projectile, in an unusual call that a BPD dispatcher had to relay to an officer twice, because the world's oldest commissioned warship is not in the habit of firing towards land and BPD isn't usually called on to help find errant projectiles.
 
This is a bit different:

Who loaded the wrong cannon? During a tour, we were told that they only fire the cannon for evening salute and it didn't face anything. Also it was a powder charge. Did someone leave the powder ram in the barrel?
So many questions.
