It's the permanent barrel length, you will measure the length of the actual barrel and not include the flash hider, brake, suppressor, etc.
The overall length should include the size of the stock fully extended (for those that have adjustable stocks).
On the M4 Carbine site they have an E-form tutorial and it says on a pinned muzzle device, its included in the barrel length, FWIW.

I just completed an eForm submission. Filled out the eForm 1 and then uploaded my NFA trust document and a copy of my MA LTC-A. Paid with a CC. Got the electronic confirmation and all looks OK provided I haven't made any errors. I've done this before without the trust (COP sign off) and that took a long time but I finally did get the stamp.
The electronic form1 does makes it pretty straightforward and easy.

It is required if the state you reside in requires you to have a license to possess the item being registered.
Yes. It will get kicked back without it.
I've been told that only one of the trustees needs to provide their LTC but I can't confirm that.
